Meditemp are recruiting for an experienced CPN (Band 6) to work in Rayleigh, Essex. The role will be working with older adults and you will act as an independent practitioner within a defined community setting, while liaising with other professionals as necessary.

The candidate will be responsible as the Care Coordinator for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers within a defined catchment area/locality. To provide professional advice and support to other agencies and members of the multidisciplinary team, To also act as a role model and resource for junior/less experienced staff.

The successful candidate will be available full time Monday – Friday 9am – 5pm 

Community Psychiatric Nurse essential skills:

• Implement evidence-based care plans and risk assessments
• The ability to implement and share best practice of local policy across multi agency platforms
• The ability to build and maintain professional and therapeutic relationships with patients and their families
• Knowledge and experience of clinical and management supervision.
• Good organisation and problem-solving skills
• Manage and communicate sensitive information effectively
• Effective team work skills

Meditemp requires you to have:

• A minimum of 6 months recent experience as a children and young person’s community psychiatric nurse
• NMC registration
• Eligibility to work within the UK
